Narrative:
On takeoff roll engine #2 flamed out at 140 KTS. Takeoff was aborted prior to V1. On taxi-in it was noted #2 fuel lever was in the off position. Tire #8 fuse plug blew after taxi-in. Upon returning to the gate, we discussed the situation with tulsa tech and station maintenance. After the required maintenance was performed and a thorough inspection of the engine and its related components was performed, and a power assurance runup was performed, the flight departed and flew routinely to jfk, with particular attention paid to the monitoring of the fuel lever. No discrepancies were noted. I was told by the tulsa technical staff that there is no history or trend of fuel lever problems on this aircraft. As a follow-up, I am requesting that I be informed whether any post event research revealed any such problems.

Title: WDB HAS ENG FLAME OUT ON TKOF. ROLL ABORT.
Narrative: ON TKOF ROLL ENG #2 FLAMED OUT AT 140 KTS. TKOF WAS ABORTED PRIOR TO V1. ON TAXI-IN IT WAS NOTED #2 FUEL LEVER WAS IN THE OFF POS. TIRE #8 FUSE PLUG BLEW AFTER TAXI-IN. UPON RETURNING TO THE GATE, WE DISCUSSED THE SIT WITH TULSA TECH AND STATION MAINT. AFTER THE REQUIRED MAINT WAS PERFORMED AND A THOROUGH INSPECTION OF THE ENG AND ITS RELATED COMPONENTS WAS PERFORMED, AND A PWR ASSURANCE RUNUP WAS PERFORMED, THE FLT DEPARTED AND FLEW ROUTINELY TO JFK, WITH PARTICULAR ATTN PAID TO THE MONITORING OF THE FUEL LEVER. NO DISCREPANCIES WERE NOTED. I WAS TOLD BY THE TULSA TECHNICAL STAFF THAT THERE IS NO HISTORY OR TREND OF FUEL LEVER PROBS ON THIS ACFT. AS A FOLLOW-UP, I AM REQUESTING THAT I BE INFORMED WHETHER ANY POST EVENT RESEARCH REVEALED ANY SUCH PROBS.
