Narrative:

A flight instructor went for a 141 assistant chief instructor chkride with the FAA. The chkride was being given at the indianapolis flight standards district office. The instructor had flown the aircraft over to indianapolis international from his/her home airport that morning. The chkride/oral started by the examiner checking through the aircraft logs to make sure that the aircraft was up to standards for the check. The examiner found a recurring ad that had not been signed off in the aircraft log at the proper time on previous inspections and had not been signed off on the most recent inspection of the aircraft. The examiner then gave the flight instructor a ferry permit to fly the aircraft back to his/her home airport. The examiner at that time also told the instructor that he would not violate him/her for the occurrence. When the flight instructor returned the aircraft back to the home airport the ad was properly (double checked) complied with and signed off in the logs. The examiner also stated that he would not violate anyone at the flight school or the mechanic who performed the work.

Title: RPTR FLEW ACFT TO CHKRIDE NOT KNOWING AN AD HAD NOT BEEN COMPLIED WITH.

Narrative: A FLT INSTRUCTOR WENT FOR A 141 ASSISTANT CHIEF INSTRUCTOR CHKRIDE WITH THE FAA. THE CHKRIDE WAS BEING GIVEN AT THE INDIANAPOLIS FLT STANDARDS DISTRICT OFFICE. THE INSTRUCTOR HAD FLOWN THE ACFT OVER TO INDIANAPOLIS INTL FROM HIS/HER HOME ARPT THAT MORNING. THE CHKRIDE/ORAL STARTED BY THE EXAMINER CHKING THROUGH THE ACFT LOGS TO MAKE SURE THAT THE ACFT WAS UP TO STANDARDS FOR THE CHK. THE EXAMINER FOUND A RECURRING AD THAT HAD NOT BEEN SIGNED OFF IN THE ACFT LOG AT THE PROPER TIME ON PREVIOUS INSPECTIONS AND HAD NOT BEEN SIGNED OFF ON THE MOST RECENT INSPECTION OF THE ACFT. THE EXAMINER THEN GAVE THE FLT INSTRUCTOR A FERRY PERMIT TO FLY THE ACFT BACK TO HIS/HER HOME ARPT. THE EXAMINER AT THAT TIME ALSO TOLD THE INSTRUCTOR THAT HE WOULD NOT VIOLATE HIM/HER FOR THE OCCURRENCE. WHEN THE FLT INSTRUCTOR RETURNED THE ACFT BACK TO THE HOME ARPT THE AD WAS PROPERLY (DOUBLE CHKED) COMPLIED WITH AND SIGNED OFF IN THE LOGS. THE EXAMINER ALSO STATED THAT HE WOULD NOT VIOLATE ANYONE AT THE FLT SCHOOL OR THE MECH WHO PERFORMED THE WORK.
