Narrative:

I was conducting an instrument evaluation in an aircraft. The acting PIC was in the left seat with the hood on. I requested a practice ILS approach and would maintain VFR. The approach controller began vectoring us for the ILS to runway 6L. The controller terminated radar about 3 mi outside the marker and said to contact the tower. The PF called the tower and said we were 3 mi west of the marker. The controller said that for a missed approach we should turn to 270 degrees. I continued. We crossed the marker and the student went through the 6 T's but did not report the marker. I waited to see if he would finally report and I got distracted looking for traffic and didn't report. On a 1/4 mi final to runway 6L, I noticed an aircraft lifting off on runway 24R. He asked the tower what to do. I directed my student to miss the approach. The 2 aircraft did not come close, but the tower got very upset. He said that we were told to report the marker inbound. I didn't recall the request, but I noticed later that he asked all aircraft on practice approachs to report. We were VFR, however, this incident could have been avoided by always informing ATC of final fix inbound. Radar was terminated so this would have been mandatory in IFR, we weren't IFR though. This field has only had a tower for a few weeks and we have been using it for practice approachs for many yrs and provided our own separation. That is why I didn't get too concerned with the traffic on runway 24. We have done training in this situation for yrs and broke off the approachs when necessary on our own without getting too close to opposing traffic. I guess the familiarity with the place and situation caused complacency about the new tower. It was also a case of letting the student go too far before taking corrective action. Supplemental information from acn 293016: I was told by the controller to contact the tower. I did this 3 mi from the final approach fix and was told to report the OM inbound. Upon obtaining the GS, I ran through the 6 T's but somehow failed to report inbound.

Title: PLT MAKES A PRACTICE APCH AND FAILS TO CALL THE TWR AT THE FINAL APCH FIX. POTENTIAL CONFLICT.

THE APCH CTLR BEGAN VECTORING US FOR THE ILS TO RWY 6L. THE CTLR TERMINATED RADAR ABOUT 3 MI OUTSIDE THE MARKER AND SAID TO CONTACT THE TWR. THE PF CALLED THE TWR AND SAID WE WERE 3 MI W OF THE MARKER. THE CTLR SAID THAT FOR A MISSED APCH WE SHOULD TURN TO 270 DEGS. I CONTINUED. WE CROSSED THE MARKER AND THE STUDENT WENT THROUGH THE 6 T'S BUT DID NOT RPT THE MARKER. I WAITED TO SEE IF HE WOULD FINALLY RPT AND I GOT DISTRACTED LOOKING FOR TFC AND DIDN'T RPT. ON A 1/4 MI FINAL TO RWY 6L, I NOTICED AN ACFT LIFTING OFF ON RWY 24R. HE ASKED THE TWR WHAT TO DO. I DIRECTED MY STUDENT TO MISS THE APCH. THE 2 ACFT DID NOT COME CLOSE, BUT THE TWR GOT VERY UPSET. HE SAID THAT WE WERE TOLD TO RPT THE MARKER INBOUND. I DIDN'T RECALL THE REQUEST, BUT I NOTICED LATER THAT HE ASKED ALL ACFT ON PRACTICE APCHS TO RPT. WE WERE VFR, HOWEVER, THIS INCIDENT COULD HAVE BEEN AVOIDED BY ALWAYS INFORMING ATC OF FINAL FIX INBOUND. RADAR WAS TERMINATED SO THIS WOULD HAVE BEEN MANDATORY IN IFR, WE WEREN'T IFR THOUGH. THIS FIELD HAS ONLY HAD A TWR FOR A FEW WKS AND WE HAVE BEEN USING IT FOR PRACTICE APCHS FOR MANY YRS AND PROVIDED OUR OWN SEPARATION. THAT IS WHY I DIDN'T GET TOO CONCERNED WITH THE TFC ON RWY 24. WE HAVE DONE TRAINING IN THIS SIT FOR YRS AND BROKE OFF THE APCHS WHEN NECESSARY ON OUR OWN WITHOUT GETTING TOO CLOSE TO OPPOSING TFC. I GUESS THE FAMILIARITY WITH THE PLACE AND SIT CAUSED COMPLACENCY ABOUT THE NEW TWR. IT WAS ALSO A CASE OF LETTING THE STUDENT GO TOO FAR BEFORE TAKING CORRECTIVE ACTION. SUPPLEMENTAL INFO FROM ACN 293016: I WAS TOLD BY THE CTLR TO CONTACT THE TWR. I DID THIS 3 MI FROM THE FINAL APCH FIX AND WAS TOLD TO RPT THE OM INBOUND. UPON OBTAINING THE GS, I RAN THROUGH THE 6 T'S BUT SOMEHOW FAILED TO RPT INBOUND.
