Narrative:

At XF30 nov/xx/94 in ponca city, ok, the other pilot and I had just boarded air carrier flight to go to dfw. I had finished the manifest and computed the center of gravity on our hand held center of gravity calculators. We were running a little ahead of schedule and instead of writing the takeoff and landing index in their appropriate boxes on the manifest, I intentionally left them blank in case a last min passenger walked up. The center of gravity was well within legal limits and, when we reached departure time, I gave the appropriate copy of the manifest to the agent and we flew to dfw. I discovered the mistake later in the day when the FAA was doing a routine station inspection. The 2 previous days I had worked 13 hour shifts and this was the beginning of my third. I can't say that I was at the peak level of performance. The pilot I was flying with and I have agreed that we will double check each other's paperwork, and I will be sure to look very carefully at my own and anybody else's paperwork that I may be flying with.

Title: FAA INSPECTOR FOUND THAT THE LTT COMMUTER AIR CARRIER FLC HAD NOT COMPLETED THE REQUIRED WT AND BAL FORM IN SOME AREAS DURING A RAMP INSPECTION.

Narrative: AT XF30 NOV/XX/94 IN PONCA CITY, OK, THE OTHER PLT AND I HAD JUST BOARDED ACR FLT TO GO TO DFW. I HAD FINISHED THE MANIFEST AND COMPUTED THE CTR OF GRAVITY ON OUR HAND HELD CTR OF GRAVITY CALCULATORS. WE WERE RUNNING A LITTLE AHEAD OF SCHEDULE AND INSTEAD OF WRITING THE TKOF AND LNDG INDEX IN THEIR APPROPRIATE BOXES ON THE MANIFEST, I INTENTIONALLY LEFT THEM BLANK IN CASE A LAST MIN PAX WALKED UP. THE CTR OF GRAVITY WAS WELL WITHIN LEGAL LIMITS AND, WHEN WE REACHED DEP TIME, I GAVE THE APPROPRIATE COPY OF THE MANIFEST TO THE AGENT AND WE FLEW TO DFW. I DISCOVERED THE MISTAKE LATER IN THE DAY WHEN THE FAA WAS DOING A ROUTINE STATION INSPECTION. THE 2 PREVIOUS DAYS I HAD WORKED 13 HR SHIFTS AND THIS WAS THE BEGINNING OF MY THIRD. I CAN'T SAY THAT I WAS AT THE PEAK LEVEL OF PERFORMANCE. THE PLT I WAS FLYING WITH AND I HAVE AGREED THAT WE WILL DOUBLE CHK EACH OTHER'S PAPERWORK, AND I WILL BE SURE TO LOOK VERY CAREFULLY AT MY OWN AND ANYBODY ELSE'S PAPERWORK THAT I MAY BE FLYING WITH.
