Narrative:

Flight was en route from rome to jfk. At about 2 hours out from jfk I felt an acute pain in the right side. A doctor was summoned ahead. I was diagnosed as having either a ruptured appendix or a kidney stone. I elected to continue to jfk. As I was almost completely incapacitated, I instructed the first officer to fly the approach from the left hand seat and the other first officer (relief officer) to fly the right seat. The plane landed at jfk without incident. I was taken by ambulance to jamaica hospital in new york where a kidney stone was diagnosed.
