Narrative:

Airlines implemented new checklists in all fleets in dec/93. These checklists were developed without any line pilot input. As a result, they are seriously flawed, and pilots have been writing on the checklists to remind them of important items that have been omitted because they are SOP. One item is the seat belt sign on the approach descent checklist. Response is to admonish crews for writing on the checklists, and no attempt is being made to rectify the omissions that cause crews to write on the checklists.

Narrative: AIRLINES IMPLEMENTED NEW CHKLISTS IN ALL FLEETS IN DEC/93. THESE CHKLISTS WERE DEVELOPED WITHOUT ANY LINE PLT INPUT. AS A RESULT, THEY ARE SERIOUSLY FLAWED, AND PLTS HAVE BEEN WRITING ON THE CHKLISTS TO REMIND THEM OF IMPORTANT ITEMS THAT HAVE BEEN OMITTED BECAUSE THEY ARE SOP. ONE ITEM IS THE SEAT BELT SIGN ON THE APCH DSCNT CHKLIST. RESPONSE IS TO ADMONISH CREWS FOR WRITING ON THE CHKLISTS, AND NO ATTEMPT IS BEING MADE TO RECTIFY THE OMISSIONS THAT CAUSE CREWS TO WRITE ON THE CHKLISTS.
