Narrative:

Approaching limmi intersection, #1 flight attendant advised captain of seriously ill passenger. 2 doctors and 1 registered nurse were located and attending passenger. Captain visited cabin to ascertain passenger condition, doctor was administering 'accuchk' for blood sugar. Captain advised doctor closest point of approach to bermuda is now and ETA is 45 mins. Doctor said he would advise in a couple of mins. Next contact with doctor was that passenger was in critical need of emergency medical care. In vicinity of swapp intersection captain turned 90 degrees off airway, descended 1000 ft to FL300, turned on all lights, declared an emergency, and broadcast intentions and position on 121.5. In approximately 2 mins new york arinc advised a revised clearance reposition direct bda but clearance based on FL310. Flight climbed back to FL310 and proceeded direct bda at best forward speed.

IN APPROX 2 MINS NEW YORK ARINC ADVISED A REVISED CLRNC REPOSITION DIRECT BDA BUT CLRNC BASED ON FL310. FLT CLBED BACK TO FL310 AND PROCEEDED DIRECT BDA AT BEST FORWARD SPD.
