Narrative:

Belligerent captain refusing to obey FARS or company procedures. The following were willful violations by the captain without any regard for my conflict with his decisions: taxiing without ATC clearance. The captain always initiated taxi without ATC authorization. Additionally, he would constantly weave, cutting corners wherever possible in his rate (route). Unauthorized admission to flight deck. The captain, during climb out at 3000 ft MSL over sfo bay, beckoned and permitted the flight attendant to access the cockpit below 10000 ft to survey the scenery. Sterile cockpit. The captain would not keep his comments strictly about operational necessities below 10000 ft. Excess speed below 10000 ft. In order to make his flight home, the captain decided to accelerate to 340 KTS at 7000 ft. Additionally, despite my resistance, he refused to comply with ATC speed instructions. During descent, at 340 KTS, ATC assigned a speed of 270 KTS. The captain 'dialed up' 290 KTS. He just grunted when questioned by me. Exceeding aircraft limitations. In his rush to get home, he selected a speed of .82M which gave us a momentary overspd warning. He failed to write up the overspd condition in the maintenance logbook. Refusal to adhere to checklist discipline. He very reluctantly calls for a checklist, so, in spite of me being the copilot, I must initiate them. When I read an item, he simply repeats the memorized response without confirming switch position. Refusal to obey company procedures. He never verifies altitude assignments, often misses radio calls, and very rarely makes required cockpit callouts. In response to my callouts, he smirks at me. My response: several times, I just about took control of the airplane, deciding I would return to the gate, if he didn't finish the checklist. I hesitated to do my duties (ie, company communications, etc) for fear that he would get me a violation. I enforced the checklist upon him. I made my callouts even though he would not respond. In short, I was flying without a captain. It's not that he is incapable, but unwilling! He will not listen to me. Sits like this are not covered adequately in cockpit resource management. My plan: to seek the advice of alpa. To show him a copy of this narrative before my next trip. If he fails to change, I will get off the airplane and notify my chief pilot. Any ideas?

Narrative: BELLIGERENT CAPT REFUSING TO OBEY FARS OR COMPANY PROCS. THE FOLLOWING WERE WILLFUL VIOLATIONS BY THE CAPT WITHOUT ANY REGARD FOR MY CONFLICT WITH HIS DECISIONS: TAXIING WITHOUT ATC CLRNC. THE CAPT ALWAYS INITIATED TAXI WITHOUT ATC AUTHORIZATION. ADDITIONALLY, HE WOULD CONSTANTLY WEAVE, CUTTING CORNERS WHEREVER POSSIBLE IN HIS RATE (RTE). UNAUTH ADMISSION TO FLT DECK. THE CAPT, DURING CLBOUT AT 3000 FT MSL OVER SFO BAY, BECKONED AND PERMITTED THE FLT ATTENDANT TO ACCESS THE COCKPIT BELOW 10000 FT TO SURVEY THE SCENERY. STERILE COCKPIT. THE CAPT WOULD NOT KEEP HIS COMMENTS STRICTLY ABOUT OPERATIONAL NECESSITIES BELOW 10000 FT. EXCESS SPD BELOW 10000 FT. IN ORDER TO MAKE HIS FLT HOME, THE CAPT DECIDED TO ACCELERATE TO 340 KTS AT 7000 FT. ADDITIONALLY, DESPITE MY RESISTANCE, HE REFUSED TO COMPLY WITH ATC SPD INSTRUCTIONS. DURING DSCNT, AT 340 KTS, ATC ASSIGNED A SPD OF 270 KTS. THE CAPT 'DIALED UP' 290 KTS. HE JUST GRUNTED WHEN QUESTIONED BY ME. EXCEEDING ACFT LIMITATIONS. IN HIS RUSH TO GET HOME, HE SELECTED A SPD OF .82M WHICH GAVE US A MOMENTARY OVERSPD WARNING. HE FAILED TO WRITE UP THE OVERSPD CONDITION IN THE MAINT LOGBOOK. REFUSAL TO ADHERE TO CHKLIST DISCIPLINE. HE VERY RELUCTANTLY CALLS FOR A CHKLIST, SO, IN SPITE OF ME BEING THE COPLT, I MUST INITIATE THEM. WHEN I READ AN ITEM, HE SIMPLY REPEATS THE MEMORIZED RESPONSE WITHOUT CONFIRMING SWITCH POS. REFUSAL TO OBEY COMPANY PROCS. HE NEVER VERIFIES ALT ASSIGNMENTS, OFTEN MISSES RADIO CALLS, AND VERY RARELY MAKES REQUIRED COCKPIT CALLOUTS. IN RESPONSE TO MY CALLOUTS, HE SMIRKS AT ME. MY RESPONSE: SEVERAL TIMES, I JUST ABOUT TOOK CTL OF THE AIRPLANE, DECIDING I WOULD RETURN TO THE GATE, IF HE DIDN'T FINISH THE CHKLIST. I HESITATED TO DO MY DUTIES (IE, COMPANY COMS, ETC) FOR FEAR THAT HE WOULD GET ME A VIOLATION. I ENFORCED THE CHKLIST UPON HIM. I MADE MY CALLOUTS EVEN THOUGH HE WOULD NOT RESPOND. IN SHORT, I WAS FLYING WITHOUT A CAPT. IT'S NOT THAT HE IS INCAPABLE, BUT UNWILLING! HE WILL NOT LISTEN TO ME. SITS LIKE THIS ARE NOT COVERED ADEQUATELY IN COCKPIT RESOURCE MGMNT. MY PLAN: TO SEEK THE ADVICE OF ALPA. TO SHOW HIM A COPY OF THIS NARRATIVE BEFORE MY NEXT TRIP. IF HE FAILS TO CHANGE, I WILL GET OFF THE AIRPLANE AND NOTIFY MY CHIEF PLT. ANY IDEAS?
