Narrative:

I authority/authorized flight crew at ind to defer maintenance of a leading edge slat indicating system which requires pulling (asymmetric mode) circuit breaker MEL 27L on jun/sat/94. Due to a flight crew refusal of aircraft at mco I had FBO mechanic accomplish an operational check of the leading edge slat indicating system and clear the deferred item. I failed to have FBO at mco reset the asymmetric mode circuit breaker. The aircraft flew in revenue service until a flight crew discovered pulled and collared circuit breaker.

Title: COMPANY GND PERSON HAD AN FBO MECH WORK ON FLAP SLAT SYS OF ACR ACFT. AN IMPORTANT CIRCUIT WAS NOT RESET AFTER THE WORK WAS COMPLETED AND THE ACFT FLEW IN SVC 5 DAYS BEFORE CIRCUIT BREAKER WAS RESET.
