Narrative:

Government X holding on final approach to runway 34 at dyess air force base between 10 mi to end of runway at 3000 ft MSL checking the ILS. Dyess tower called requesting a release on military Y, t- 38, opposite direction. Government X was approximately 7 mi final inbound. Dyess tower was told to release Y runway heading to cross 5 mi south at or above 5000 ft when the flight check aircraft turns left to start his sbound run. Government X was 5 mi final when dyess tower reported military Y rolling. Approach immediately called dyess back to abort Y takeoff but Y was between frequencys. Approach advised X of the traffic coming at him and he reported the aircraft in sight. The 2 crossed each other within 3 mi of the runway. We were later advised that dyess tower had mentioned the traffic to Y prior to departure but Y never reported the traffic in sight. Dyess tower continually neglects to look out of the tower windows to check on traffic prior to issuing control instructions to other aircraft.

DYESS TWR WAS TOLD TO RELEASE Y RWY HDG TO CROSS 5 MI S AT OR ABOVE 5000 FT WHEN THE FLT CHK ACFT TURNS L TO START HIS SBOUND RUN. GOV X WAS 5 MI FINAL WHEN DYESS TWR RPTED MIL Y ROLLING. APCH IMMEDIATELY CALLED DYESS BACK TO ABORT Y TKOF BUT Y WAS BTWN FREQS. APCH ADVISED X OF THE TFC COMING AT HIM AND HE RPTED THE ACFT IN SIGHT. THE 2 CROSSED EACH OTHER WITHIN 3 MI OF THE RWY. WE WERE LATER ADVISED THAT DYESS TWR HAD MENTIONED THE TFC TO Y PRIOR TO DEP BUT Y NEVER RPTED THE TFC IN SIGHT. DYESS TWR CONTINUALLY NEGLECTS TO LOOK OUT OF THE TWR WINDOWS TO CHK ON TFC PRIOR TO ISSUING CTL INSTRUCTIONS TO OTHER ACFT.
