Narrative:

Recently, schenectady control tower was evaluated by washington evaluations branch. One problem area identified was our incident/accident reporting procedure, and that some incidents or violations have been stopped by schenectady county airport commissioner, which is true. An evaluator ordered alb qats to deliver FAA form 8020.11 to our facility for formal reporting. Airport management has not made this form available for use by the tower staff, even though they have had this form for approximately 2 months. Additionally, the manager had reported to the FAA that schenectady has no areas of non visibility, which is not true, as we have several areas. I was not allowed access to any information on these subjects for ATC input. The new tower chief and airport manager has discontinued the use of flight progress strips, which is mandatory, according to FAA manuals. Callback conversation with reporter revealed the following information: reporter was informed if ASRS telcon'ed the reporter might be identified by the content of the report. Reporter was advised if he wanted to go further with the non-compliance with FAA regulations by the airport manager he could call the administrators hot line. Reporter said he only sent in the report to cover himself with the FAA because of the washington evaluation.

Title: ARPT MGMNT NON COMPLIANCE WITH FAA REGULATIONS FAILURE TO USE FORMAL INCIDENT RPTING FORM.

Narrative: RECENTLY, SCHENECTADY CTL TWR WAS EVALUATED BY WASHINGTON EVALUATIONS BRANCH. ONE PROB AREA IDENTIFIED WAS OUR INCIDENT/ACCIDENT RPTING PROC, AND THAT SOME INCIDENTS OR VIOLATIONS HAVE BEEN STOPPED BY SCHENECTADY COUNTY ARPT COMMISSIONER, WHICH IS TRUE. AN EVALUATOR ORDERED ALB QATS TO DELIVER FAA FORM 8020.11 TO OUR FACILITY FOR FORMAL RPTING. ARPT MGMNT HAS NOT MADE THIS FORM AVAILABLE FOR USE BY THE TWR STAFF, EVEN THOUGH THEY HAVE HAD THIS FORM FOR APPROX 2 MONTHS. ADDITIONALLY, THE MGR HAD RPTED TO THE FAA THAT SCHENECTADY HAS NO AREAS OF NON VISIBILITY, WHICH IS NOT TRUE, AS WE HAVE SEVERAL AREAS. I WAS NOT ALLOWED ACCESS TO ANY INFO ON THESE SUBJECTS FOR ATC INPUT. THE NEW TWR CHIEF AND ARPT MGR HAS DISCONTINUED THE USE OF FLT PROGRESS STRIPS, WHICH IS MANDATORY, ACCORDING TO FAA MANUALS. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR WAS INFORMED IF ASRS TELCON'ED THE RPTR MIGHT BE IDENTIFIED BY THE CONTENT OF THE RPT. RPTR WAS ADVISED IF HE WANTED TO GO FURTHER WITH THE NON-COMPLIANCE WITH FAA REGULATIONS BY THE ARPT MGR HE COULD CALL THE ADMINISTRATORS HOT LINE. RPTR SAID HE ONLY SENT IN THE RPT TO COVER HIMSELF WITH THE FAA BECAUSE OF THE WASHINGTON EVALUATION.
