Narrative:
Small aircraft X PA30 departed without clearance, while on an IFR flight plan. This was the pilot deviation. After departing, he turned into the path of an arriving air carrier Y DH-8. Y turned to avoid the PA30. This was the near miss. Y said he would file paperwork on the PA30, as was nqx approach control.

Narrative: SMA X PA30 DEPARTED WITHOUT CLRNC, WHILE ON AN IFR FLT PLAN. THIS WAS THE PLTDEV. AFTER DEPARTING, HE TURNED INTO THE PATH OF AN ARRIVING ACR Y DH-8. Y TURNED TO AVOID THE PA30. THIS WAS THE NEAR MISS. Y SAID HE WOULD FILE PAPERWORK ON THE PA30, AS WAS NQX APCH CTL.
