Narrative:

It had been a very long day, with minimum (9 hours) rest from the night before. The trip to green bay had been a schedule change, so we were late to arrive at green bay (about 2 hours late). We had no passenger going out, so we needed to add ballast for the weight and balance. The station is minimum staffed. So I left the aircraft to help the ramp personnel load the ballast. While I was out of the cockpit the first officer performed the before start checklist. We had not been fueled yet, but when he reached fuel quantity on the checklist he made a mental note to get fuel rather than holding the checklist at that point, or placing the fuel quantity on the 'skip list.' when I returned to the cockpit he had finished the checklists and weight and balance, and we were ready to start the engines. It wasn't until after takeoff that I noticed we didn't have the proper fuel. We returned to be fueled. I will now always make sure both cockpit crew members are present when the checklists are performed, and the checklist is held or the item placed on the skip list when an item cannot be completed at that point.

Title: FLC TECHNIQUE PREFLT PROC DOES NOT DISCLOSE THEIR FAILURE TO ADD FUEL PRIOR TO DEP.

Narrative: IT HAD BEEN A VERY LONG DAY, WITH MINIMUM (9 HRS) REST FROM THE NIGHT BEFORE. THE TRIP TO GREEN BAY HAD BEEN A SCHEDULE CHANGE, SO WE WERE LATE TO ARRIVE AT GREEN BAY (ABOUT 2 HRS LATE). WE HAD NO PAX GOING OUT, SO WE NEEDED TO ADD BALLAST FOR THE WT AND BAL. THE STATION IS MINIMUM STAFFED. SO I LEFT THE ACFT TO HELP THE RAMP PERSONNEL LOAD THE BALLAST. WHILE I WAS OUT OF THE COCKPIT THE FO PERFORMED THE BEFORE START CHKLIST. WE HAD NOT BEEN FUELED YET, BUT WHEN HE REACHED FUEL QUANTITY ON THE CHKLIST HE MADE A MENTAL NOTE TO GET FUEL RATHER THAN HOLDING THE CHKLIST AT THAT POINT, OR PLACING THE FUEL QUANTITY ON THE 'SKIP LIST.' WHEN I RETURNED TO THE COCKPIT HE HAD FINISHED THE CHKLISTS AND WT AND BAL, AND WE WERE READY TO START THE ENGS. IT WASN'T UNTIL AFTER TKOF THAT I NOTICED WE DIDN'T HAVE THE PROPER FUEL. WE RETURNED TO BE FUELED. I WILL NOW ALWAYS MAKE SURE BOTH COCKPIT CREW MEMBERS ARE PRESENT WHEN THE CHKLISTS ARE PERFORMED, AND THE CHKLIST IS HELD OR THE ITEM PLACED ON THE SKIP LIST WHEN AN ITEM CANNOT BE COMPLETED AT THAT POINT.
