Narrative:

After takeoff aircraft would not pressurize. Checklists were run, all system checked ok, so we returned to slc. An uneventful overweight landing was made and we returned to the gate. The forward entry door was open just a crack on the aft side. The door system did not indicate this condition nor did the agent or pushback crews detect the problem. The flight attendants noticed extra noise on taxi but didn't judge it loud enough to be reported to the cockpit. Neither pilot noticed elevated noise levels. A post-flight overweight inspection was completed. We all need to be more aware of normal/abnormal conditions. Any of 4 agencies (gate, pushback crew, flight attendants, or pilots) would have noticed something was not quite right before takeoff power was applied.

Title: FLC OF AN LGT ACR ACFT RETURNED AND LANDED AFTER ACFT WOULD NOT PRESSURIZE AFTER TKOF DUE THE MAIN CABIN DOOR NOT PROPERLY CLOSED.

Narrative: AFTER TKOF ACFT WOULD NOT PRESSURIZE. CHKLISTS WERE RUN, ALL SYS CHKED OK, SO WE RETURNED TO SLC. AN UNEVENTFUL OVERWT LNDG WAS MADE AND WE RETURNED TO THE GATE. THE FORWARD ENTRY DOOR WAS OPEN JUST A CRACK ON THE AFT SIDE. THE DOOR SYS DID NOT INDICATE THIS CONDITION NOR DID THE AGENT OR PUSHBACK CREWS DETECT THE PROB. THE FLT ATTENDANTS NOTICED EXTRA NOISE ON TAXI BUT DIDN'T JUDGE IT LOUD ENOUGH TO BE RPTED TO THE COCKPIT. NEITHER PLT NOTICED ELEVATED NOISE LEVELS. A POST-FLT OVERWT INSPECTION WAS COMPLETED. WE ALL NEED TO BE MORE AWARE OF NORMAL/ABNORMAL CONDITIONS.
