Narrative:

During my work shift I was assigned to do a transit check on a medium large transport aircraft. In the course of the night I replaced the #1 main wheel and tire assembly due to wear and forgot to enter the tire change in the logbook. I do not think there is anything I can say or change to prevent a recurrence. I just had a mental breakdown in forgetting to enter it in the logbook. P.south., when I got home and realized it happened, I called my foreman and I believe the tire assembly was again changed down line.

Title: MECH FAILS TO MAKE LOGBOOK ENTRY.

Narrative: DURING MY WORK SHIFT I WAS ASSIGNED TO DO A TRANSIT CHK ON A MLG ACFT. IN THE COURSE OF THE NIGHT I REPLACED THE #1 MAIN WHEEL AND TIRE ASSEMBLY DUE TO WEAR AND FORGOT TO ENTER THE TIRE CHANGE IN THE LOGBOOK.
