Narrative:

During flight sfo-cdg, center hydraulic system quantity began to leak. Flight diverted to jfk for maintenance. During divert and with ATC approval, fuel dumped from C tank over ohio to reduce weight towards maximum landing weight. During dump, overhead annunciator light showed left dump valve not opened. Flight crew member went aft and verified both dump valves opened and dumping. Fuel dumped and aircraft landed at jfk by me 1500 pounds overweight. During maintenance we, including me, advised maintenance in maintenance office of overweight landing. They said they would do proper inspection. When departing jfk, told mechanic we forgot to write up overweight landing and he said not to worry, inspection already completed and recorded. Then during flight jfk-cdg we realized we'd not written up the dump valve indicator light malfunction. We wrote this up going into paris. Both valves dumped so we weren't quite as concerned as we should have been. We fully intended to write it up but the remainder of the first leg with reduced hydraulics and other concerns apparently put it to a lower priority in our minds and we didn't think of it again until jfk-cdg. Technically, the overweight should have been written up also, but maintenance personnel indicated no need as the inspection was already completed and recorded. Supplemental information from acn 252223: in the dump we noticed the left valve light on. I went back to check it. It was working properly. When we landed in jfk we had to put the gear and flaps down with alternate system. Maintenance said we wouldn't have to but we did. After landing, system was about 17 percent of normal. We got the leak fixed and proceeded to cdg, forgetting to write up the dump valve light because valve worked, it just indicated it didn't. We got to paris. We told the outbound crew and they said the dump valve light was a no-go item, so by the time they got it fixed, they were out of duty time so they cancelled the flight.

Narrative: DURING FLT SFO-CDG, CTR HYD SYS QUANTITY BEGAN TO LEAK. FLT DIVERTED TO JFK FOR MAINT. WE FULLY INTENDED TO WRITE IT UP BUT THE REMAINDER OF THE FIRST LEG WITH REDUCED HYDS AND OTHER CONCERNS APPARENTLY PUT IT TO A LOWER PRIORITY IN OUR MINDS AND WE DIDN'T THINK OF IT AGAIN UNTIL JFK-CDG. TECHNICALLY, THE OVERWT SHOULD HAVE BEEN WRITTEN UP ALSO, BUT MAINT PERSONNEL INDICATED NO NEED AS THE INSPECTION WAS ALREADY COMPLETED AND RECORDED. SUPPLEMENTAL INFORMATION FROM ACN 252223: IN THE DUMP WE NOTICED THE L VALVE LIGHT ON. I WENT BACK TO CHK IT. IT WAS WORKING PROPERLY. WHEN WE LANDED IN JFK WE HAD TO PUT THE GEAR AND FLAPS DOWN WITH ALTERNATE SYS. MAINT SAID WE WOULDN'T HAVE TO BUT WE DID. AFTER LNDG, SYS WAS ABOUT 17 PERCENT OF NORMAL. WE GOT THE LEAK FIXED AND PROCEEDED TO CDG, FORGETTING TO WRITE UP THE DUMP VALVE LIGHT BECAUSE VALVE WORKED, IT JUST INDICATED IT DIDN'T. WE GOT TO PARIS. WE TOLD THE OUTBOUND CREW AND THEY SAID THE DUMP VALVE LIGHT WAS A NO-GO ITEM, SO BY THE TIME THEY GOT IT FIXED, THEY WERE OUT OF DUTY TIME SO THEY CANCELLED THE FLT.
