Narrative:

Air carrier X was cleared for takeoff on a closed runway. All preventive measures were in use. There were no personnel or equipment on the runway, however, the lights were not activated. Human error is the only contributing factor. There was 1 other aircraft on frequency, landing opposite direction and it is common to depart on the parallel runway. Callback conversation with reporter revealed the following information: reporter stated that she issued taxi instructions to air carrier X. Runway was closed after air carrier X began taxi and reporter did not inform air carrier X. Reporter stated she just went brain dead. Reporter experience 5 yrs non radar.

Title: ACR X TKOF ON CLOSED RWY. SYS ERROR.

Narrative: ACR X WAS CLRED FOR TKOF ON A CLOSED RWY. ALL PREVENTIVE MEASURES WERE IN USE. THERE WERE NO PERSONNEL OR EQUIP ON THE RWY, HOWEVER, THE LIGHTS WERE NOT ACTIVATED. HUMAN ERROR IS THE ONLY CONTRIBUTING FACTOR. THERE WAS 1 OTHER ACFT ON FREQ, LNDG OPPOSITE DIRECTION AND IT IS COMMON TO DEPART ON THE PARALLEL RWY. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATED THAT SHE ISSUED TAXI INSTRUCTIONS TO ACR X. RWY WAS CLOSED AFTER ACR X BEGAN TAXI AND RPTR DID NOT INFORM ACR X. RPTR STATED SHE JUST WENT BRAIN DEAD. RPTR EXPERIENCE 5 YRS NON RADAR.
