Narrative:
Ord tower cleared our aircraft into 'position and hold' 9L at ord. Per company procedure, cabin PA must be made. Somehow, captain was not on tower, and my radio volume was off during PA, which I made. After several seconds, we re-established communication with tower, who had cleared us twice for takeoff, which we had not heard.

Narrative: ORD TWR CLRED OUR ACFT INTO 'POS AND HOLD' 9L AT ORD. PER COMPANY PROC, CABIN PA MUST BE MADE. SOMEHOW, CAPT WAS NOT ON TWR, AND MY RADIO VOLUME WAS OFF DURING PA, WHICH I MADE. AFTER SEVERAL SECONDS, WE RE-ESTABLISHED COM WITH TWR, WHO HAD CLRED US TWICE FOR TKOF, WHICH WE HAD NOT HEARD.
