Narrative:

Cleared by chicago approach for ILS approach runway 16 at pwk approximately 3 mi east of northbrook VOR. Contacted tower and instructed to fly right hand traffic runway 30R. Lowered gear and observed 2 green lights (main gear is a single mechanical unit, hence only 2 indicators). On final approach airspeed 90 KTS, completely retarded power at 150 ft AGL. No gear warning horn was heard. Main gear touched runway simultaneously. Lowered nose and propeller. Impacted runway. Nose impacted and aircraft slid down centerline approximately 600-700 ft before coming to a stop. Preliminary inspection indicates propeller beyond repair. Undetermined engine damage due to sudden stop and both lower and upper cowlings damaged. All damage appears forward of firewall. No injuries incurred by pilot.

LOWERED GEAR AND OBSERVED 2 GREEN LIGHTS (MAIN GEAR IS A SINGLE MECHANICAL UNIT, HENCE ONLY 2 INDICATORS). ON FINAL APCH AIRSPD 90 KTS, COMPLETELY RETARDED PWR AT 150 FT AGL. NO GEAR WARNING HORN WAS HEARD. MAIN GEAR TOUCHED RWY SIMULTANEOUSLY. LOWERED NOSE AND PROP. IMPACTED RWY. NOSE IMPACTED AND ACFT SLID DOWN CTRLINE APPROX 600-700 FT BEFORE COMING TO A STOP. PRELIMINARY INSPECTION INDICATES PROP BEYOND REPAIR. UNDETERMINED ENG DAMAGE DUE TO SUDDEN STOP AND BOTH LOWER AND UPPER COWLINGS DAMAGED. ALL DAMAGE APPEARS FORWARD OF FIREWALL.
