Narrative:

Enclosed is a report detailing a violation of an FAA regulation discovered as part of an internal program evaluation conducted by a part 135 air carrier based at monroe custer airport in monroe, mi. An identical report has been submitted to the FAA's detroit FSDO pursuant to FAA advisory circular 120- 56, dated jan/xx/92, which pertains to air carrier voluntary disclosure of far violations. Between jan/xx/93 and jan/xy/93, a part 135 pilot, mr. X, attended a flight training seminar conducted by FBO. At the request of them, mr. X completed a form which detailed the number of flight hours he had amassed in flying turboprop aircraft. It is the air carrier's belief that training unit may have granted mr. X a waiver from the actual check flight requirement for an light transport type aircraft based on the number of flight hours mr. X reported he had completed. As a result, mr. X obtained a 'type' rating for the light transport based solely on his experience with a phase ii simulator. On may/xx/93, we discovered that mr. X may have misstated the number of flight hours in turboprop aircraft he reported to air carrier. Because of the misstatement, mr. X may not have had the minimum number of flight hours (in the preceding 12 months) necessary in order to waive the check flight requirement for the light transport aircraft. Upon information and belief, mr. X may have violated FAA check flight guidelines during the period from jan/xx/93 to may/xx/93 on each occasion that he flew the light transport aircraft. This disclosure covers and includes any such flts by mr. X. Mr. X has been immediately suspended from flying any aircraft for which he has not been check flight 'type' certified and was immediately suspended as a pilot by air carrier.

BTWN JAN/XX/93 AND JAN/XY/93, A PART 135 PLT, MR. X, ATTENDED A FLT TRAINING SEMINAR CONDUCTED BY FBO. AT THE REQUEST OF THEM, MR. X MAY HAVE MISSTATED THE NUMBER OF FLT HRS IN TURBOPROP ACFT HE RPTED TO ACR. BECAUSE OF THE MISSTATEMENT, MR. X MAY NOT HAVE HAD THE MINIMUM NUMBER OF FLT HRS (IN THE PRECEDING 12 MONTHS) NECESSARY IN ORDER TO WAIVE THE CHK FLT REQUIREMENT FOR THE LTT ACFT. UPON INFO AND BELIEF, MR. X MAY HAVE VIOLATED FAA CHK FLT GUIDELINES DURING THE PERIOD FROM JAN/XX/93 TO MAY/XX/93 ON EACH OCCASION THAT HE FLEW THE LTT ACFT. THIS DISCLOSURE COVERS AND INCLUDES ANY SUCH FLTS BY MR. X. MR. X HAS BEEN IMMEDIATELY SUSPENDED FROM FLYING ANY ACFT FOR WHICH HE HAS NOT BEEN CHK FLT 'TYPE' CERTIFIED AND WAS IMMEDIATELY SUSPENDED AS A PLT BY ACR.
