Narrative:

Aircraft experienced an in-flight shutdown of #1 engine. According to company records I was responsible for servicing the oil quantity on #1 engine the night before, therefore, I am at fault. The oil cap was found off laying in the scupper of the oil tank. I must assume it popped off in-flight or was mistakenly left off. Company is reprimanding me by 2 days off unpaid. I would like to see some type of device installed on the oil service doors to disallow closing the door with the cap off or not properly latched. In the future, be assured, I will check these caps carefully.

Title: LGT FLC FORCED TO SHUT DOWN ENG DUE OIL LOSS. CAP NOT REPLACED AFTER MAINT.

Narrative: ACFT EXPERIENCED AN INFLT SHUTDOWN OF #1 ENG. ACCORDING TO COMPANY RECORDS I WAS RESPONSIBLE FOR SVCING THE OIL QUANTITY ON #1 ENG THE NIGHT BEFORE, THEREFORE, I AM AT FAULT. THE OIL CAP WAS FOUND OFF LAYING IN THE SCUPPER OF THE OIL TANK. I MUST ASSUME IT POPPED OFF INFLT OR WAS MISTAKENLY LEFT OFF. COMPANY IS REPRIMANDING ME BY 2 DAYS OFF UNPAID. I WOULD LIKE TO SEE SOME TYPE OF DEVICE INSTALLED ON THE OIL SVC DOORS TO DISALLOW CLOSING THE DOOR WITH THE CAP OFF OR NOT PROPERLY LATCHED. IN THE FUTURE, BE ASSURED, I WILL CHK THESE CAPS CAREFULLY.
