Narrative:

On mar/thu/93, I was captain of flight from mdw to pit. Because of a snow flurry, I arranged to deice. The deice crew performed a preliminary (but inadequate) application at the gate. They said they would complete the job after pushback. Pushback was normal with communication conducted between me and the ground crewman via interphone. After the brakes were parked, I said, 'company policy requires that when deicing is completed, for you to give me the time and your payroll number for our records.' soon after, he said, 'time is AM45, solution 50/50, type I. My payroll number is xxxxx.' he then disconnected from the aircraft, waved me off, and walked back to the terminal. When I landed in pit, I was met by my flight manager who informed me that mdw charged that I had started both engines and taxied out with crew still deicing tail of aircraft. If so, there is no way I could have known beyond using normal procedures and communication.

Title: ODD LEVELS OF GND FLC COORD EXHIBITED IN PREFLT PROC DEICING ON ACR MLG.

Narrative: ON MAR/THU/93, I WAS CAPT OF FLT FROM MDW TO PIT. BECAUSE OF A SNOW FLURRY, I ARRANGED TO DEICE. THE DEICE CREW PERFORMED A PRELIMINARY (BUT INADEQUATE) APPLICATION AT THE GATE. THEY SAID THEY WOULD COMPLETE THE JOB AFTER PUSHBACK. PUSHBACK WAS NORMAL WITH COM CONDUCTED BTWN ME AND THE GND CREWMAN VIA INTERPHONE. AFTER THE BRAKES WERE PARKED, I SAID, 'COMPANY POLICY REQUIRES THAT WHEN DEICING IS COMPLETED, FOR YOU TO GIVE ME THE TIME AND YOUR PAYROLL NUMBER FOR OUR RECORDS.' SOON AFTER, HE SAID, 'TIME IS AM45, SOLUTION 50/50, TYPE I. MY PAYROLL NUMBER IS XXXXX.' HE THEN DISCONNECTED FROM THE ACFT, WAVED ME OFF, AND WALKED BACK TO THE TERMINAL. WHEN I LANDED IN PIT, I WAS MET BY MY FLT MGR WHO INFORMED ME THAT MDW CHARGED THAT I HAD STARTED BOTH ENGS AND TAXIED OUT WITH CREW STILL DEICING TAIL OF ACFT. IF SO, THERE IS NO WAY I COULD HAVE KNOWN BEYOND USING NORMAL PROCS AND COM.
