Narrative:

Aircraft pushed off gate to deice. Ground crew requested to disconnect for deice and use hand signals. Captain requested ground crew hook back up for engine starts. Ground crew didn't want to due to cold WX but finally agreed. Once deice completed engine starts initiated. Ground crew, unhappy about having to hook back up, did not comment on starts and sent tug back to gate prior to engine starts being accomplished. Ground crew then moved under aircraft out of view of captain. After start checks completed. Taxi clearance requested and received. Turned nosewheel and added power. Ground crewman moved to be seen by captain and requested to disconnect. Removal of tug prior to ground crew disconnect was non standard and broke the normal chain of events designed to ensure everyone's safety. Supplemental information from acn 227354: aircraft needed deicing. Deice crew did not advise prior to starting deice procedure. Crew advised when complete. This was not in accordance with procedures of airline and did not allow flight crew to accurately measure time elapsed from deice to takeoff to ensure deice fluid still effective. Better communications and adherence to airline procedures are a must. Flight crew completed a visual inspection of wings prior to taxi.
