Narrative:

This report is reluctantly submitted upon request of the chief pilot in accordance with company operations manual. Flight X pit to jhw. Saturday md/92. Captain's report regarding flight attendant unprofessional and unsafe behavior. Crew: captain, first officer, flight attendant. Charges: neglecting duties. Example, failure to prepare cabin for next flight. Failure to perform cabin preflight inspection upon aircraft swaps. This is total disregard for my certificate as well as company's. Insubordination. Example, failure to clean cabin when directed. Failure to exit cockpit as directed by captain, in violation of FARS. Undermining captain authority. Example: privately advising first officer of: 1. Composing 5 page incriminating letter of captain for management. 2. Impending attrition in lyh allowing him to move up. Additionally, instructing other company operations personnel to tell the captain to 'clean his own cabin.' intimidation and interference with flight crew. Far violation. Example, loudly advising captain to talk to (chief pilot) if he has a problem with herself. Meanwhile, rapidly entering the cockpit with outrage. Both engines were operating. Unsafe practice. Example, failure to advise captain of impending illness. Failure to properly stow/display emergency exit briefing cards in seats. This flight attendant maintains an adversary position with the flight crew, especially the first officer's. She is an impediment to total crew harmony. Her slovenly cabin habits are unacceptable and an embarrassment to the flight crew. Future discrepancies in performance will be brought to the attention of company management as directed. I thank all of my colleagues for their extra efforts during this flight attendant's escapade. Recommendation: stand with and support your capts as you want to be supported. Please do not allow this to begin the erosion of the captain as final authority.

FAR VIOLATION. EXAMPLE, LOUDLY ADVISING CAPT TO TALK TO (CHIEF PLT) IF HE HAS A PROBLEM WITH HERSELF. MEANWHILE, RAPIDLY ENTERING THE COCKPIT WITH OUTRAGE. BOTH ENGS WERE OPERATING. UNSAFE PRACTICE. EXAMPLE, FAILURE TO ADVISE CAPT OF IMPENDING ILLNESS. FAILURE TO PROPERLY STOW/DISPLAY EMER EXIT BRIEFING CARDS IN SEATS. THIS FLT ATTENDANT MAINTAINS AN ADVERSARY POS WITH THE FLC, ESPECIALLY THE FO'S. SHE IS AN IMPEDIMENT TO TOTAL CREW HARMONY. HER SLOVENLY CABIN HABITS ARE UNACCEPTABLE AND AN EMBARRASSMENT TO THE FLC. FUTURE DISCREPANCIES IN PERFORMANCE WILL BE BROUGHT TO THE ATTN OF COMPANY MGMNT AS DIRECTED. I THANK ALL OF MY COLLEAGUES FOR THEIR EXTRA EFFORTS DURING THIS FLT ATTENDANT'S ESCAPADE. RECOMMENDATION: STAND WITH AND SUPPORT YOUR CAPTS AS YOU WANT TO BE SUPPORTED. PLEASE DO NOT ALLOW THIS TO BEGIN THE EROSION OF THE CAPT AS FINAL AUTHORITY.
