Narrative:

Approximately 3 hours into the flight (XA30Z), after captain had eaten dinner, he put on his oxygen mask and stayed on it for about 4 or 5 mins. He removed his mask and said he wasn't feeling well and was going to go to the blue room and get rid of some food. About 10 mins later first flight attendant came to the cockpit and told us he was sick and she had summoned his wife who was on the trip with him. When his condition worsened, flight attendant found doctor on board, and he told us captain was very ill and asked me if there would be a problem landing the plane without him. I assured him that would not be a problem and so gave him the medical kit so he could give captain a shot to ease his pain. I told so to take left seat and monitor the flight engineers panel from the left seat. He contacted flight control in atlanta to let them know of the problem. I asked flight attendant to see if any of the non-revenue passengers were pilots but after checking the cabin there weren't any on board. I told her that we would need a flight attendant in the second officers seat for the approach and landing and she volunteered. She turned over her duties in the cabin to another qualified faic and briefed her on the problem. When flight attendant finished in the cabin she came to the cockpit and so briefed her on the so panel. I briefed them on the approach and landing and notified ZLA that we had a very ill crew member and would like the approach expedited and to have paramedics meet the airplane at the gate. The flight landed without incident and captain was taken to the hospital by the paramedics.
