Narrative:

I was instructed by mlu approach control to contact mlu tower when I was on a 1 1/2 mi right base leg for runway 18. Upon contacting the tower, I was informed that I was #2 behind another small aircraft who was on a midfield left downwind for runway 18. I had to enter an extended right downwind and visually locate the #1 traffic. There was another small aircraft approximately 2 or 3 mi northeast of my position who was advised by the tower that he was #3 for runway 18. While visually locating the 2 aircraft, I became distracted from my checklist. See diagram on reverse side.

Title: CPR PLT OF SMA ACFT LANDED GEAR-UP.

Narrative: I WAS INSTRUCTED BY MLU APCH CTL TO CONTACT MLU TWR WHEN I WAS ON A 1 1/2 MI R BASE LEG FOR RWY 18. UPON CONTACTING THE TWR, I WAS INFORMED THAT I WAS #2 BEHIND ANOTHER SMA WHO WAS ON A MIDFIELD L DOWNWIND FOR RWY 18. I HAD TO ENTER AN EXTENDED R DOWNWIND AND VISUALLY LOCATE THE #1 TFC. THERE WAS ANOTHER SMA APPROX 2 OR 3 MI NE OF MY POS WHO WAS ADVISED BY THE TWR THAT HE WAS #3 FOR RWY 18. WHILE VISUALLY LOCATING THE 2 ACFT, I BECAME DISTRACTED FROM MY CHKLIST.
