Narrative:

We were cleared for takeoff on runway 04R at lafayette regional airport, lafayette, la. As we accelerated, high and low areas in the runway became obvious. It was a rough ride down the runway. That runway has a lot of patches in it and I really had to hold on to my nosewheel tiller. It was terribly rough. Aircraft jumping up and down. At 1 point we actually stopped accelerating according to the airspeed indicator. That runway is in terrible shape. After liftoff I advised the tower about the terrible condition of runway 04R. No answer!!! Not their problem I guess! They heard me, told me to contact departure control. I'm trying to avoid takeoff on runway 04R.
