Small transport X turned opposite direction with small aircraft Y. (Both at 7000 ft). Conflict alert went off. Evasive action taken by controller's aircraft were in process of being switched. (Communication at radar control). Previous controller did not brief relieving controller of non standard coordination with adjacent controller. Small aircraft Y standard operation is to be at 5000. Previous controller accepted radar handoff. Radar controller should have seen opposite direction traffic at 7000. Traffic was on his radar tag. Receiving controller should have asked what the plan with small transport X was. Knowing he had to turn opposite direction 7000 ft. Better communication needed to assure safe operation.

BETTER COM NEEDED TO ASSURE SAFE OP.
