Narrative:

As a reasonable prudent person, my understanding of far 67, as pertaining to medical questions on form. I may have not properly answered the question on dizziness as I was treated from 1981-1987 for vertigo, from an automatic accident when I was hit in the rear of my car by another car in may 1981. I have sued that individual for expenses. From 1987-1992 I am unsure of the proper way to address medical questions and answering as I have had no symptoms and I am not being treated versus statute of limitation? In any event, I am filing this form for the years 1981 through 1992 inclusions pending possible violation on question far 67.
