Narrative:

*Captain greatly displeased by attitude of first officer alleging first officer using foot and shoe to operate heading bug on panel. Lax indifferent attitude with relaxed posture in cockpit with feet up on instrument panel reading paper in-flight. Improperly flown approach to iad despite captain's attempt to brief the approach and advise that a procedure turn was required. First officer became insubordinate and abusive in demeanor. During debriefing of approach first officer was argumentative and unamendable to suggestion or advice. During the approach the right engine went into reversion which first officer ignored and left the aircraft on autoplt even when it was obvious the autoplt could not handle the yaw. Engine reversion was improperly handled by first officer. Captain determined first officer should be removed from duty pending further investigation.
