Narrative:

I departed eagles' nest aerodrome (FD44) to flagler county airport (X47) for the purpose of topping off fuel tanks for an early morning IFR departure from eagles' nest. Total fuel added at flagler county airport was 53.7 gallons. Tank capacity is 142 gallons. Departure from flagler involved a short taxi to departure runway, run up normal, takeoff normal. Insight graphic engine monitors had just been installed and had been test flown the 2 preceding days on 2 30-min flts and had checked out normally. After level off I first noticed that the cht temperatures on the insight instruments were giving readings that were not covered in the insight manual. Also the readings were erratic. About the same time the alternator fail light illuminated. Checked that out and was not able to restore the alternator. I don't believe that was part of the problem. I next checked the old cht gauges and found the temperatures way above normal. By this time I was over home base and landed normally. Flight time 15 mins. About an hour later, the fueling manager at flagler called to see if I had arrived safely. He then advised me that I had been fueled with jet fuel. After talking to the accident prevention program manager for the area on nov/xx/91, I learned there were 8 aircraft that had been misfueled. I don't believe there was any way to have detected this misfueling in this case except for smelling the fuel and/or draining fuel onto your hand, maybe not even then. It appears that many standard procedures for accepting delivery of fuel had not been followed. There is no jet fuel sold nor stored at this airport. As far as I know at this time a tanker truck picked up the jet fuel at a distribution point and was put in the 100 ll undergnd tank at the airport. Callback conversation with reporter revealed the following: reporter states the manufacturer says engines must be completely torn down and every part inspected. Consequently, he has been unable to fly his aircraft since the incident. Scenario of events leading to the problem is as follows: airport called their source for fuel and ordered 100 ll. Jax did not have 100 ll at that time. They called another source to deliver to flagler airport. The driver assigned had only delivered jet fuel in his time with the company. He filled his truck with jet fuel even though the order stated 100 ll. Delivered to airport and in spite of color coded cap on undergnd tank, pumped jet fuel into 100 ll tank. Driver took paperwork into office and gave them delivery sheet for 100 ll fuel. 4 insurance companies are currently involved in trying to settle the situation. Reporter will not rebuild and use these engines. He says he could never be comfortable with them.

Title: SMA FUELED WITH JET FUEL INSTEAD OF AVIATION GAS.

Narrative: I DEPARTED EAGLES' NEST AERODROME (FD44) TO FLAGLER COUNTY ARPT (X47) FOR THE PURPOSE OF TOPPING OFF FUEL TANKS FOR AN EARLY MORNING IFR DEP FROM EAGLES' NEST. TOTAL FUEL ADDED AT FLAGLER COUNTY ARPT WAS 53.7 GALLONS. TANK CAPACITY IS 142 GALLONS. DEP FROM FLAGLER INVOLVED A SHORT TAXI TO DEP RWY, RUN UP NORMAL, TKOF NORMAL. INSIGHT GRAPHIC ENG MONITORS HAD JUST BEEN INSTALLED AND HAD BEEN TEST FLOWN THE 2 PRECEDING DAYS ON 2 30-MIN FLTS AND HAD CHKED OUT NORMALLY. AFTER LEVEL OFF I FIRST NOTICED THAT THE CHT TEMPS ON THE INSIGHT INSTS WERE GIVING READINGS THAT WERE NOT COVERED IN THE INSIGHT MANUAL. ALSO THE READINGS WERE ERRATIC. ABOUT THE SAME TIME THE ALTERNATOR FAIL LIGHT ILLUMINATED. CHKED THAT OUT AND WAS NOT ABLE TO RESTORE THE ALTERNATOR. I DON'T BELIEVE THAT WAS PART OF THE PROBLEM. I NEXT CHKED THE OLD CHT GAUGES AND FOUND THE TEMPS WAY ABOVE NORMAL. BY THIS TIME I WAS OVER HOME BASE AND LANDED NORMALLY. FLT TIME 15 MINS. ABOUT AN HR LATER, THE FUELING MGR AT FLAGLER CALLED TO SEE IF I HAD ARRIVED SAFELY. HE THEN ADVISED ME THAT I HAD BEEN FUELED WITH JET FUEL. AFTER TALKING TO THE ACCIDENT PREVENTION PROGRAM MGR FOR THE AREA ON NOV/XX/91, I LEARNED THERE WERE 8 ACFT THAT HAD BEEN MISFUELED. I DON'T BELIEVE THERE WAS ANY WAY TO HAVE DETECTED THIS MISFUELING IN THIS CASE EXCEPT FOR SMELLING THE FUEL AND/OR DRAINING FUEL ONTO YOUR HAND, MAYBE NOT EVEN THEN. IT APPEARS THAT MANY STANDARD PROCS FOR ACCEPTING DELIVERY OF FUEL HAD NOT BEEN FOLLOWED. THERE IS NO JET FUEL SOLD NOR STORED AT THIS ARPT. AS FAR AS I KNOW AT THIS TIME A TANKER TRUCK PICKED UP THE JET FUEL AT A DISTRIBUTION POINT AND WAS PUT IN THE 100 LL UNDERGND TANK AT THE ARPT. CALLBACK CONVERSATION WITH REPORTER REVEALED THE FOLLOWING: RPTR STATES THE MANUFACTURER SAYS ENGS MUST BE COMPLETELY TORN DOWN AND EVERY PART INSPECTED. CONSEQUENTLY, HE HAS BEEN UNABLE TO FLY HIS ACFT SINCE THE INCIDENT. SCENARIO OF EVENTS LEADING TO THE PROBLEM IS AS FOLLOWS: ARPT CALLED THEIR SOURCE FOR FUEL AND ORDERED 100 LL. JAX DID NOT HAVE 100 LL AT THAT TIME. THEY CALLED ANOTHER SOURCE TO DELIVER TO FLAGLER ARPT. THE DRIVER ASSIGNED HAD ONLY DELIVERED JET FUEL IN HIS TIME WITH THE COMPANY. HE FILLED HIS TRUCK WITH JET FUEL EVEN THOUGH THE ORDER STATED 100 LL. DELIVERED TO ARPT AND IN SPITE OF COLOR CODED CAP ON UNDERGND TANK, PUMPED JET FUEL INTO 100 LL TANK. DRIVER TOOK PAPERWORK INTO OFFICE AND GAVE THEM DELIVERY SHEET FOR 100 LL FUEL. 4 INSURANCE COMPANIES ARE CURRENTLY INVOLVED IN TRYING TO SETTLE THE SITUATION. RPTR WILL NOT REBUILD AND USE THESE ENGS. HE SAYS HE COULD NEVER BE COMFORTABLE WITH THEM.
