Narrative:
When filling out weight and balance I fill out the weight and balance and the captain initials it too. Rushed to fill out the form, I filled out the form wrong and later the FAA ramp checked the flight's paperwork. Basically, the whole form was filled out wrong. This particular form was different and for a loner aircraft from another airliner on which the weight and balance was very different. Not familiar with this form and rushed and new in the widebody transport I filled it out wrong.

Title: FO OF CHARTER WDB FILLED OUT THE WT AND BAL FORM INCORRECTLY AND THE ERROR WAS CAUGHT BY AN ACI DOING AN ACFT RAMP CHK.
Narrative: WHEN FILLING OUT WT AND BAL I FILL OUT THE WT AND BAL AND THE CAPT INITIALS IT TOO. RUSHED TO FILL OUT THE FORM, I FILLED OUT THE FORM WRONG AND LATER THE FAA RAMP CHKED THE FLT'S PAPERWORK. BASICALLY, THE WHOLE FORM WAS FILLED OUT WRONG. THIS PARTICULAR FORM WAS DIFFERENT AND FOR A LONER ACFT FROM ANOTHER AIRLINER ON WHICH THE WT AND BAL WAS VERY DIFFERENT. NOT FAMILIAR WITH THIS FORM AND RUSHED AND NEW IN THE WDB I FILLED IT OUT WRONG.
