Narrative:

I was working local control. I cleared an air carrier X for takeoff on runway 31 and visually scanned the runway. He turned the corner, started takeoff roll, and as he began to rotate an small aircraft Y crossed the runway downfield on taxiway G. Callback conversation with reporter revealed the following information: reporter experience 5 yrs radar. Reporter stated the ground controller (who was a developmental controller verified on the ground control position) had taxied air carrier X to an inactive runway for departure at air carrier X request. The ground controller released the runway to the local controller but forgot he had cleared small aircraft Y to cross the runway earlier. Air carrier X rotated and missed small aircraft Y by 300 ft horizontal.

CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR EXPERIENCE 5 YRS RADAR. RPTR STATED THE GND CTLR (WHO WAS A DEVELOPMENTAL CTLR VERIFIED ON THE GND CTL POS) HAD TAXIED ACR X TO AN INACTIVE RWY FOR DEP AT ACR X REQUEST. THE GND CTLR RELEASED THE RWY TO THE LCL CTLR BUT FORGOT HE HAD CLRED SMA Y TO CROSS THE RWY EARLIER. ACR X ROTATED AND MISSED SMA Y BY 300 FT HORIZ.
