Narrative:

My student was on his first supervised solo. While taxiing to the runway he was confused by the controllers direction and taxied onto the active runway. After finally making it to the runup he called for takeoff. When he called ready on 19L he stepped on a guy ready on 19R. When he released the microphone all he heard was 'cleared for takeoff.' as he taxied onto the runway he asked for closed traffic and without a reply took off without a clearance. He knew to ask if he was in doubt yet never asked for help even when he admits the instructions didn't make sense. To make matters worse there was a FSDO inspector in the tower supervising. I'm sure the largest contributing factor was his nervousness. I doubt any action will be taken against me, however they did take down my certificate number and name.

Title: SPI MADE FIRST SOLO TKOF WITHOUT TWR CLRNC.

Narrative: MY STUDENT WAS ON HIS FIRST SUPERVISED SOLO. WHILE TAXIING TO THE RWY HE WAS CONFUSED BY THE CTLRS DIRECTION AND TAXIED ONTO THE ACTIVE RWY. AFTER FINALLY MAKING IT TO THE RUNUP HE CALLED FOR TKOF. WHEN HE CALLED READY ON 19L HE STEPPED ON A GUY READY ON 19R. WHEN HE RELEASED THE MIC ALL HE HEARD WAS 'CLRED FOR TKOF.' AS HE TAXIED ONTO THE RWY HE ASKED FOR CLOSED TFC AND WITHOUT A REPLY TOOK OFF WITHOUT A CLRNC. HE KNEW TO ASK IF HE WAS IN DOUBT YET NEVER ASKED FOR HELP EVEN WHEN HE ADMITS THE INSTRUCTIONS DIDN'T MAKE SENSE. TO MAKE MATTERS WORSE THERE WAS A FSDO INSPECTOR IN THE TWR SUPERVISING. I'M SURE THE LARGEST CONTRIBUTING FACTOR WAS HIS NERVOUSNESS. I DOUBT ANY ACTION WILL BE TAKEN AGAINST ME, HOWEVER THEY DID TAKE DOWN MY CERTIFICATE NUMBER AND NAME.
