Narrative:

In fwa we landed on runway 23 and exited the runway on taxiway C3. While the captain was taxiing off the runway I noticed that the intersection was not as depicted on the approach plate #11-1. Upon further inspection I noticed several errors on the chart as opposed to how the taxiways were actually constructed. I have enclosed a copy of the approach plate and marked the areas of concern. The parallel red slashed indicate where the taxiway does not continue but in fact is broken by a ditch. In the area of C3 the diagram shows an east/west taxiway. This area now has taxiway lights off of it and it is closed. This depiction error could lead to an aircraft possibly taxiing onto an unsafe portion of taxiway and possibly across a closed section. These areas would be hard to see in low visibility conditions especially.

Title: FO OF ACR NOTES ERROR ON COMMERCIAL ARPT PAGE REGARDING TAXIWAY PRESENTATION.

Narrative: IN FWA WE LANDED ON RWY 23 AND EXITED THE RWY ON TAXIWAY C3. WHILE THE CAPT WAS TAXIING OFF THE RWY I NOTICED THAT THE INTXN WAS NOT AS DEPICTED ON THE APCH PLATE #11-1. UPON FURTHER INSPECTION I NOTICED SEVERAL ERRORS ON THE CHART AS OPPOSED TO HOW THE TAXIWAYS WERE ACTUALLY CONSTRUCTED. I HAVE ENCLOSED A COPY OF THE APCH PLATE AND MARKED THE AREAS OF CONCERN. THE PARALLEL RED SLASHED INDICATE WHERE THE TAXIWAY DOES NOT CONTINUE BUT IN FACT IS BROKEN BY A DITCH. IN THE AREA OF C3 THE DIAGRAM SHOWS AN E/W TAXIWAY. THIS AREA NOW HAS TAXIWAY LIGHTS OFF OF IT AND IT IS CLOSED. THIS DEPICTION ERROR COULD LEAD TO AN ACFT POSSIBLY TAXIING ONTO AN UNSAFE PORTION OF TAXIWAY AND POSSIBLY ACROSS A CLOSED SECTION. THESE AREAS WOULD BE HARD TO SEE IN LOW VISIBILITY CONDITIONS ESPECIALLY.
