Narrative:

Did not realize how tired and fatigued I was after agreeing to a 30 minute fdp (flight duty period) extension until we were on the takeoff roll above 100 knots and I had called 'V1' and then I started to call 'rotate' as the airplane passed through the V1 bug a full 10 knots below our planned rotation speed that was bugged at 139 knots. I had briefed the speeds with the captain on the before takeoff checklist not two minutes prior. I also failed to catch that the approach mode was not armed for the approach as we flew through the glide slope intercept.
