Dangerous goods stacked on top of dangerous goods; causing cardboard boxes to be pressed down and go out of shape. Dangerous goods stacked on top of dangerous goods should be avoided; if there is a possibility of goods can press underlying boxes to be pressed out of shape and causing holes. Build-up and loading personnel not paying proper attention to correct dangerous goods build-up procedures.
