Narrative:

Clearance was iah 4 departure with an initial level off altitude of 400' MSL. On readback we read only the transponder code which was acknowledged by clearance delivery. After takeoff aircraft was leveled at 4000'. Departure control said we were only cleared to 3000', which we did not hear in the original clearance or any subsequent clearance. ATC requests readback of only transponder code in clearance. Any altitude changes to an SID should require transponder code and those changes to be read back.
