Narrative:

Ground controller instructed pilot to taxi to runway 36 for back taxi. Pilot taxied onto and began back taxiing on runway 36. The ground controller stopped the aircraft just as he approached runway 28, the active arrival runway. Other aircraft were on the ILS 28 approach but I can't ascertain their position as traverse city is only a level ii VFR tower. The same controller has had 2 operrors within the past 3 weeks as far as I can determine no action has been taken to correct the problem. If the ground controller used correct phraseology and watched his traffic taxiing I believe that he wouldn't have had either error. My major concern is that management is not reporting these errors as required and are covering these up.

Title: SMA TAXIED ONTO ACTIVE RWY BEFORE HOLDING POSITION RESULTING IN LESS THAN STANDARD SEPARATION.

Narrative: GND CTLR INSTRUCTED PLT TO TAXI TO RWY 36 FOR BACK TAXI. PLT TAXIED ONTO AND BEGAN BACK TAXIING ON RWY 36. THE GND CTLR STOPPED THE ACFT JUST AS HE APCHED RWY 28, THE ACTIVE ARR RWY. OTHER ACFT WERE ON THE ILS 28 APCH BUT I CAN'T ASCERTAIN THEIR POS AS TRAVERSE CITY IS ONLY A LEVEL II VFR TWR. THE SAME CTLR HAS HAD 2 OPERRORS WITHIN THE PAST 3 WKS AS FAR AS I CAN DETERMINE NO ACTION HAS BEEN TAKEN TO CORRECT THE PROB. IF THE GND CTLR USED CORRECT PHRASEOLOGY AND WATCHED HIS TFC TAXIING I BELIEVE THAT HE WOULDN'T HAVE HAD EITHER ERROR. MY MAJOR CONCERN IS THAT MGMNT IS NOT RPTING THESE ERRORS AS REQUIRED AND ARE COVERING THESE UP.
