Narrative:

We departed shv with said clearance. Departure communication was normal. Upon reaching 8400' MSL, controller asked why we were not level at 5000'. Exact words: 'air carrier X, I show you at 8400 and you were to level off at 5000; say intentions.' we informed him that we were going to 12000', as originally cleared. Another problem occurred when somehow equipment was changed to a (aircraft type). Equipment is always filed as a (type). Also, controllers informed us that our final destination was btr, and not msy. All controllers involved were informed of the wrong in-flight.

Title: CTLR QUESTIONS ALT ACR CLIMBING TO.

Narrative: WE DEPARTED SHV WITH SAID CLRNC. DEP COM WAS NORMAL. UPON REACHING 8400' MSL, CTLR ASKED WHY WE WERE NOT LEVEL AT 5000'. EXACT WORDS: 'ACR X, I SHOW YOU AT 8400 AND YOU WERE TO LEVEL OFF AT 5000; SAY INTENTIONS.' WE INFORMED HIM THAT WE WERE GOING TO 12000', AS ORIGINALLY CLRED. ANOTHER PROB OCCURRED WHEN SOMEHOW EQUIP WAS CHANGED TO A (ACFT TYPE). EQUIP IS ALWAYS FILED AS A (TYPE). ALSO, CTLRS INFORMED US THAT OUR FINAL DEST WAS BTR, AND NOT MSY. ALL CTLRS INVOLVED WERE INFORMED OF THE WRONG INFLT.
