Narrative:

On flight from ewr to pvd, I was PNF. I was giving our ETA to company, passenger announcement to passenger, getting ATIS and picking landing weight for airspeed bugs. We were approximately 100 mi (20 mins) from landing at pvd. PF crossed bridge port VOR and never turned to fly arwy. My VOR was on destination for reference. Copilot VOR on bridge port and due to no DME and very small to/from triangles on VOR, he missed turn. I was task saturated and was out of the monitor picture. This is not unusual on 2-M crews. When 1 pilot distracted or saturated with duty items, other pilot is left alone, with no backup. DME on the VOR station and bigger to/from VOR flags would help. Also this was second night of a 3 day trip. Our first trip was a bna overnight arriving at 1 am. Second night our arrival time was 11 pm body time. Although this trip was not that bad, we constantly tell our company about onerous trips and reaction is very slow to help the problem. Also, our company is on a massive counseling program for sick leave usage. Grown men have to go in and be counseled if they use so many days in a give period. Airline companies keep increasing duty time and days on duty and provide less support than in the past, and have fewer and more inexperienced crew men in the cockpit. Combine this was an outdated ATC system and the future looks interesting.

Title: ACR CREW FAILS TO TURN ONTO AIRWAY AFTER CROSSING VOR.

Narrative: ON FLT FROM EWR TO PVD, I WAS PNF. I WAS GIVING OUR ETA TO COMPANY, PAX ANNOUNCEMENT TO PAX, GETTING ATIS AND PICKING LNDG WT FOR AIRSPD BUGS. WE WERE APPROX 100 MI (20 MINS) FROM LNDG AT PVD. PF CROSSED BRIDGE PORT VOR AND NEVER TURNED TO FLY ARWY. MY VOR WAS ON DEST FOR REF. COPLT VOR ON BRIDGE PORT AND DUE TO NO DME AND VERY SMALL TO/FROM TRIANGLES ON VOR, HE MISSED TURN. I WAS TASK SATURATED AND WAS OUT OF THE MONITOR PICTURE. THIS IS NOT UNUSUAL ON 2-M CREWS. WHEN 1 PLT DISTRACTED OR SATURATED WITH DUTY ITEMS, OTHER PLT IS LEFT ALONE, WITH NO BACKUP. DME ON THE VOR STATION AND BIGGER TO/FROM VOR FLAGS WOULD HELP. ALSO THIS WAS SECOND NIGHT OF A 3 DAY TRIP. OUR FIRST TRIP WAS A BNA OVERNIGHT ARRIVING AT 1 AM. SECOND NIGHT OUR ARR TIME WAS 11 PM BODY TIME. ALTHOUGH THIS TRIP WAS NOT THAT BAD, WE CONSTANTLY TELL OUR COMPANY ABOUT ONEROUS TRIPS AND REACTION IS VERY SLOW TO HELP THE PROB. ALSO, OUR COMPANY IS ON A MASSIVE COUNSELING PROGRAM FOR SICK LEAVE USAGE. GROWN MEN HAVE TO GO IN AND BE COUNSELED IF THEY USE SO MANY DAYS IN A GIVE PERIOD. AIRLINE COMPANIES KEEP INCREASING DUTY TIME AND DAYS ON DUTY AND PROVIDE LESS SUPPORT THAN IN THE PAST, AND HAVE FEWER AND MORE INEXPERIENCED CREW MEN IN THE COCKPIT. COMBINE THIS WAS AN OUTDATED ATC SYS AND THE FUTURE LOOKS INTERESTING.
