Narrative:

I was the lead. When I first pulled up the flight I saw 2 animals and a hazmat on the dangerous goods stage and to secure it with 15 bags. I pulled up my paperwork; not knowing that the load put the hazmat below the line; and I told my bag match guy to make sure to put 15 bags around it. They put in 21 bags. I proceeded to load the rest of the bag cans while they were doing [that]. I locked out my flight and it took off. Then the cargo people asked me about the hazmat and I told them it was on the plane and they had to turn the plane around and bring it back to remove it which I did. The hazmat was 13 pounds over weight.
