Narrative:

As per your directive, during your phone conversation with director of operations in 4/90, I am sending the following report. As the president of ZZZ helicopter's, incorporated, I direct the financial activities and assist the management staff as necessary. Since I am not an far 135 qualified pilot, I hired mr. X in 12/87 to be the director of operations for this company. When mr. X was hired, he understood the duties and responsibilities of his position. He also accepted his position knowing that he was responsible for this day to day operations to include safety, training, liaison with the FAA and meeting the requirements F the far's. On 10/88, mr. Y was hired by me to fill the position of director of maintenance. Mr. Y was represented to me by mr. X as his brother-in-law and a well qualified pilot and a & P mechanic with an inspection authorization. I first met mr. Y in 9/89. He came presenting a buyer for a helicopter that the company was selling. At that time I questioned him on his aviation background, and found he was qualified in far 135 operations. In 1/90 I felt that there should be more management activities in maintaining the requirements of our far 135 operations. At this time I asked mr. Q if he could come to greenville to look over our 135 operations. Mr. Q, in not knowing our day to day operations, looked over the management duties and responsibilities in the company's operations manual. Prior to leaving, mr. Q gave me a list of changes I agreed with mr. Q assessment. I addressed these changes with mr. X, but they were not well received. In 2/90 I asked mr. Q to come to work of the company as chief pilot and to assist mr. X in the operations of our company. This development was also not well received by mr. X, and he resigned in 2/90. In 2/90 mr. Y informed me that the annual on our small aircraft had been completed and that the main rotor blades for that helicopter would need to be replaced in about 100 hours. A few days later mr. Y informed me that he wanted to get out of aviation and he was resigning from the company. In 3/90 I was informed by mr. Q and the V.P. Of the company that the maintenance requirements for our small aircraft were not being met. At that time I ordered both of our helicopter's grounded and all components, parts and paperwork be brought up to correct standards. Today, both helicopter's and their components are being maintained to FAA and the manufacturer's standards. We have also established forms to keep these helicopter's up to standards.

Title: CHARTER SMA HELICOPTER PART 135 OPERATION IS CONTINUED FOR APPROX 2 YEARS WITHOUT PROPER COMPLIANCE WITH FEDERAL AVIATION REG MAINTENANCE ACTIVITY OR MAINTENANCE RECORDS.

Q, IN NOT KNOWING OUR DAY TO DAY OPS, LOOKED OVER THE MGMNT DUTIES AND RESPONSIBILITIES IN THE COMPANY'S OPS MANUAL. PRIOR TO LEAVING, MR. Q GAVE ME A LIST OF CHANGES I AGREED WITH MR. Q ASSESSMENT. I ADDRESSED THESE CHANGES WITH MR. X, BUT THEY WERE NOT WELL RECEIVED. IN 2/90 I ASKED MR. Q TO COME TO WORK OF THE COMPANY AS CHIEF PLT AND TO ASSIST MR. X IN THE OPS OF OUR COMPANY. THIS DEVELOPMENT WAS ALSO NOT WELL RECEIVED BY MR. X, AND HE RESIGNED IN 2/90. IN 2/90 MR. Y INFORMED ME THAT THE ANNUAL ON OUR SMA HAD BEEN COMPLETED AND THAT THE MAIN ROTOR BLADES FOR THAT HELI WOULD NEED TO BE REPLACED IN ABOUT 100 HRS. A FEW DAYS LATER MR. Y INFORMED ME THAT HE WANTED TO GET OUT OF AVIATION AND HE WAS RESIGNING FROM THE COMPANY. IN 3/90 I WAS INFORMED BY MR. Q AND THE V.P. OF THE COMPANY THAT THE MAINT REQUIREMENTS FOR OUR SMA WERE NOT BEING MET. AT THAT TIME I ORDERED BOTH OF OUR HELI'S GNDED AND ALL COMPONENTS, PARTS AND PAPERWORK BE BROUGHT UP TO CORRECT STANDARDS. TODAY, BOTH HELI'S AND THEIR COMPONENTS ARE BEING MAINTAINED TO FAA AND THE MANUFACTURER'S STANDARDS. WE HAVE ALSO ESTABLISHED FORMS TO KEEP THESE HELI'S UP TO STANDARDS.
