Narrative:

In 3/90 aircraft XXXX had the #2 engine hydromechanical metering unit (hmu) changed for parameter fluctuations. The hmu is rigged on the bench prior to installation on the engine. After installation on the right-hand engine, the runs proved to require further rigging of the hmu. After numerous adjustments and engine runs, the hmu had is final rigging. Operations and leak checked good. I finished the safety wiring of the arm adjustments. I also thought I had cotter pinned the 3 linkages connected to the hmu. I then had the inspector xx inspect my work. I then filled out a non-routine work card abc as compliance of work completed. I then had xy review the paperwork so he could sign off the discrepancy on logbook dgf item G and sign the airworthiness release for item G, since I am not yet authority/authorized to sign a logbook discrepancy. On 4/sun/90 aircraft XXXX landed at lch. Upon landing the aircraft had no beta light or reverse. The aft landed west/O any other incident. Maintenance was dispatched from iah to lch to troubleshoot the discrepancy. Upon inspection of the right-hand engine it was found that the linkage from the hmu to PCU at the hmu was disconnected. It appeared that I had missed the cotter pin at the hmu connection. This unfortunate incident has raised my consciousness to a higher degree for maintenance practices. The counseling any training I have received I feel have ensured that this type of oversight will not occur in the future.

Narrative: IN 3/90 ACFT XXXX HAD THE #2 ENG HYDROMECHANICAL METERING UNIT (HMU) CHANGED FOR PARAMETER FLUCTUATIONS. THE HMU IS RIGGED ON THE BENCH PRIOR TO INSTALLATION ON THE ENG. I THEN HAD XY REVIEW THE PAPERWORK SO HE COULD SIGN OFF THE DISCREPANCY ON LOGBOOK DGF ITEM G AND SIGN THE AIRWORTHINESS RELEASE FOR ITEM G, SINCE I AM NOT YET AUTH TO SIGN A LOGBOOK DISCREPANCY. ON 4/SUN/90 ACFT XXXX LANDED AT LCH. UPON LNDG THE ACFT HAD NO BETA LIGHT OR REVERSE. THE AFT LANDED W/O ANY OTHER INCIDENT. MAINT WAS DISPATCHED FROM IAH TO LCH TO TROUBLESHOOT THE DISCREPANCY. UPON INSPECTION OF THE RIGHT-HAND ENG IT WAS FOUND THAT THE LINKAGE FROM THE HMU TO PCU AT THE HMU WAS DISCONNECTED. IT APPEARED THAT I HAD MISSED THE COTTER PIN AT THE HMU CONNECTION. THIS UNFORTUNATE INCIDENT HAS RAISED MY CONSCIOUSNESS TO A HIGHER DEG FOR MAINT PRACTICES. THE COUNSELING ANY TRNING I HAVE RECEIVED I FEEL HAVE ENSURED THAT THIS TYPE OF OVERSIGHT WILL NOT OCCUR IN THE FUTURE.
