Narrative:

Mlt X helicopter took off from taxiway C at sfo SVFR to the north. He had been cleared SVFR direct hunters point, northwest of sfo. Air carrier Y took off runway 1L on an offshore SID, initial heading 340 degrees. I radar idented mlt X I mi wxw of sfo. About 20 seconds later I cleared air carrier Z, an small transport, for takeoff runway 1R, initial heading 030 degrees. Mlt X, instead of intercepting the bay shoreline or highway 101, continued to turn eastbound. As air carrier Y was 1 mi north of the airport, mlt X reported IMC and requested an approach. I turned air carrier Y to heading 010 degrees to avoid mlt X and told mlt X to hold his position. He continued on his eastbound heading and climbed. Air carrier Y passed mlt X and I told him to turn left heading 340 degrees and resume the offshore 1 departure san marcos transition. He complied and I switched him to bay departure. I told mlt X to fly heading 070 degrees and maintain 2000', as I thought the tops were about 1500' (they were about 1300'). The radar coordinator at sfo alerted me to a conflict with air carrier Z. I started to call Z (3 mi northeast sfo) and stopped, believing I had already transferred communications to bay departure. I issued traffic to mlt X, who reported VFR on top. Radar coordinator handed mlt X to bay and pointed him out to appropriate sectors. I switched mlt X to bay. I called air carrier Z again and he answered. I switched him to bay, as there was no longer a conflict. Mlt X missed air carrier Y by no more than 1/2 mi horizontal and no more than 400' vertical. He missed air carrier Z by less than that. I believe that mlt X violated his clearance by: 1) allowing himself to become IMC, 2) issuing himself an eastbound heading across the departure corridor, and 3) not holding his position when so instructed. I believe I could have issued a heading to mlt X in plenty of time to miss air carrier Z if I had been more aware of that impending conflict.

I TURNED ACR Y TO HDG 010 DEGS TO AVOID MLT X AND TOLD MLT X TO HOLD HIS POS. HE CONTINUED ON HIS EBND HDG AND CLBED. ACR Y PASSED MLT X AND I TOLD HIM TO TURN LEFT HDG 340 DEGS AND RESUME THE OFFSHORE 1 DEP SAN MARCOS TRANSITION. HE COMPLIED AND I SWITCHED HIM TO BAY DEP. I TOLD MLT X TO FLY HDG 070 DEGS AND MAINTAIN 2000', AS I THOUGHT THE TOPS WERE ABOUT 1500' (THEY WERE ABOUT 1300'). THE RADAR COORDINATOR AT SFO ALERTED ME TO A CONFLICT WITH ACR Z. I STARTED TO CALL Z (3 MI NE SFO) AND STOPPED, BELIEVING I HAD ALREADY TRANSFERRED COMS TO BAY DEP. I ISSUED TFC TO MLT X, WHO RPTED VFR ON TOP. RADAR COORDINATOR HANDED MLT X TO BAY AND POINTED HIM OUT TO APPROPRIATE SECTORS. I SWITCHED MLT X TO BAY. I CALLED ACR Z AGAIN AND HE ANSWERED. I SWITCHED HIM TO BAY, AS THERE WAS NO LONGER A CONFLICT. MLT X MISSED ACR Y BY NO MORE THAN 1/2 MI HORIZ AND NO MORE THAN 400' VERT. HE MISSED ACR Z BY LESS THAN THAT. I BELIEVE THAT MLT X VIOLATED HIS CLRNC BY: 1) ALLOWING HIMSELF TO BECOME IMC, 2) ISSUING HIMSELF AN EBND HDG ACROSS THE DEP CORRIDOR, AND 3) NOT HOLDING HIS POS WHEN SO INSTRUCTED. I BELIEVE I COULD HAVE ISSUED A HDG TO MLT X IN PLENTY OF TIME TO MISS ACR Z IF I HAD BEEN MORE AWARE OF THAT IMPENDING CONFLICT.
