Narrative:

I missed an ATC assigned crossing restriction. We were told to cross cofax intersection at 17000' on J152. Cofax intersection is 40 DME east of johnstown VORTAC on J152. Using the high altitude 4 chart, I looked at the chart and added up the DME mileage of 22 DME and 40 DME and came up with 62 DME for cofax intersection. I was very tired at the time and made an honest human error. We were eastbound from chicago to phl at that time. I called the ATC center supervisor from philadelphia (ZNY) and told him it was my fault completely and what I had done, and he said not to worry, that there was no traffic conflict and that the 'snitch machine' went off.

Title: FLT CREW OF ACR MLG MISSES CROSSING RESTRICTION AT COFAX INTERSECTION DUE TO MISREADING OF DME DISTANCE FROM CHART.

Narrative: I MISSED AN ATC ASSIGNED XING RESTRICTION. WE WERE TOLD TO CROSS COFAX INTXN AT 17000' ON J152. COFAX INTXN IS 40 DME E OF JOHNSTOWN VORTAC ON J152. USING THE HIGH ALT 4 CHART, I LOOKED AT THE CHART AND ADDED UP THE DME MILEAGE OF 22 DME AND 40 DME AND CAME UP WITH 62 DME FOR COFAX INTXN. I WAS VERY TIRED AT THE TIME AND MADE AN HONEST HUMAN ERROR. WE WERE EBND FROM CHICAGO TO PHL AT THAT TIME. I CALLED THE ATC CENTER SUPVR FROM PHILADELPHIA (ZNY) AND TOLD HIM IT WAS MY FAULT COMPLETELY AND WHAT I HAD DONE, AND HE SAID NOT TO WORRY, THAT THERE WAS NO TFC CONFLICT AND THAT THE 'SNITCH MACHINE' WENT OFF.
