Narrative:

I had been giving out for 1+10, then worked alone. I never received a briefing initially from trnee. An aircraft called ready for departure. There was no strip on him so I cleared him for takeoff. (I thought we were VFR and with no strip he must be VFR). While the aircraft was upwind I was told he taxied out west/O calling ground and was on a ground hold to destination. I then questioned the aircraft and he said he would return to the ground. I was then informed the airport was IFR. I then in the confusion issued the aircraft a SVFR clearance west/O the pilot asking or prior coordination. Another aircraft called outside the marker. I got on the override wne to talk to radar and saw the aircraft's bottom below the clouds. I ineffectively informed radar of the aircraft, but coordination a 090 degree heading to maintain 3000'. I issued it to the aircraft. The aircraft outside marker called in and I gave him a go around. I insured the departure was on a 090 degree heading and further insured sep by giving the other aircraft a 270 degree heading. Aircraft taxied west/O clearance and was not told of ground hold. Upon being relieved shortly after the incident, I informed my supervisor of my lack of awareness about the WX. I have requested a review board to look into the causal factors involved.

Title: TWR CTLR ASSIGNS A DEP ACFT A SVFR CLRNC WITHOUT PLT REQUEST, THEN CLEARS ACFT INTO CLOUDS.

Narrative: I HAD BEEN GIVING OUT FOR 1+10, THEN WORKED ALONE. I NEVER RECEIVED A BRIEFING INITIALLY FROM TRNEE. AN ACFT CALLED READY FOR DEP. THERE WAS NO STRIP ON HIM SO I CLRED HIM FOR TKOF. (I THOUGHT WE WERE VFR AND WITH NO STRIP HE MUST BE VFR). WHILE THE ACFT WAS UPWIND I WAS TOLD HE TAXIED OUT W/O CALLING GND AND WAS ON A GND HOLD TO DEST. I THEN QUESTIONED THE ACFT AND HE SAID HE WOULD RETURN TO THE GND. I WAS THEN INFORMED THE ARPT WAS IFR. I THEN IN THE CONFUSION ISSUED THE ACFT A SVFR CLRNC W/O THE PLT ASKING OR PRIOR COORD.
