Narrative:

We were descending on the arrival into ZZZ with the xxr transition. Right around the zzzzz intersection; the EICAS indicated pitch trim fail in conjunction with the caution chime. Shortly after this occurred; the autopilot disconnected and then we got the ap fail caution message as well. At this point; PF (ca) took control of flying the aircraft and the pm (first officer) ran the QRH for pitch trim fail. At this time; ATC wanted us to continue the arrival but we said we had to work through something and needed to stay on this heading and altitude. We ran the QRH and tried to reset the pitch system but the message still persisted and our pitch trim was indeed failed. The pitch trim failed in the descent so it failed at roughly 2.0 nose trim up. Being the PF; the plane required heavy back pressure to keep the plane level. ATC got crash and fire rescue available and the first officer briefed the fas and updated them. We then proceeded [to the airport] and landed. Upon landing; we taxied off the runway and proceeded to our gate. At this time we told ATC that we no longer needed any assistance. Also at this time; we got another cautionary message of flight control no dispatch. We got to the gate and parked. We made sure the fas and passengers were okay. We accomplished all of our post flight duties and contacted dispatch and maintenance to proceed with further action.

Title: EMB-175 Captain reported receiving a PITCH TRIM FAIL EICAS alert on descent. They continued the approach to a safe landing and noted that heavy back pressure was required to keep the aircraft level.
