Narrative:

I cleared small aircraft X for takeoff runway 34 at palwaukee IFR 320 degree heading maintain 30 thousand. I told small aircraft X to contact departure. Approximately 4 mins later small aircraft X called level at 30 over the VOR. I told small aircraft X to contact departure. He did and there was a systems error with IFR inbound aircraft Y. I was not getting primary targets on tower BRITE scope (equipment). Pilot did not take up assigned heading or turn on transponder. I departed a second IFR aircraft. O'hare departure talked to 2ND aircraft about 2 mins before small aircraft Y got over the VOR. Ord didn't wonder or ask where first released departure was (ord training in progress). I was also having headset problems, and the ARTS 303A acquiring of aircraft was slow. If the equipment had been working properly I/or ord controller could have seen the system error and done something to correct it.

Narrative: I CLRED SMA X FOR TKOF RWY 34 AT PALWAUKEE IFR 320 DEG HDG MAINTAIN 30 THOUSAND. I TOLD SMA X TO CONTACT DEP. APPROX 4 MINS LATER SMA X CALLED LEVEL AT 30 OVER THE VOR. I TOLD SMA X TO CONTACT DEP. HE DID AND THERE WAS A SYSTEMS ERROR WITH IFR INBND ACFT Y. I WAS NOT GETTING PRIMARY TARGETS ON TWR BRITE SCOPE (EQUIPMENT). PLT DID NOT TAKE UP ASSIGNED HDG OR TURN ON XPONDER. I DEPARTED A SECOND IFR ACFT. O'HARE DEP TALKED TO 2ND ACFT ABOUT 2 MINS BEFORE SMA Y GOT OVER THE VOR. ORD DIDN'T WONDER OR ASK WHERE FIRST RELEASED DEP WAS (ORD TRAINING IN PROGRESS). I WAS ALSO HAVING HEADSET PROBLEMS, AND THE ARTS 303A ACQUIRING OF ACFT WAS SLOW. IF THE EQUIPMENT HAD BEEN WORKING PROPERLY I/OR ORD CTLR COULD HAVE SEEN THE SYSTEM ERROR AND DONE SOMETHING TO CORRECT IT.
