Narrative:

Small aircraft X, row-cos-ioc-fnl, route of flight. I took an automated handoff from cos approach. When small aircraft X checked in he reported at 11,000, however, the strip indicated 12,000. I called cos approach and they advised that publication approach had changed the altitude, but failed to enter the information into the computer. Two approach controllers worked the aircraft, without forwarding the correct altitude via computer or to me via landline. This is a violation of handbook 7110.65.
